In Home Support

In-Home Support are available to families with children with development disabilities. With this service Integrity' Direct Support Professional Staff will come directly to your home to provide individualized support and training.

Integrity Health Care staff understand that each person and family has different needs. Our staff are culturally sensitive and will work with you and other team members to develop and implement a support plan that makes sense to you.

In-Home Support Services may include:
  • Going to places of interests in your community
  • Participating in recreation and leisure activities
  • Developing communication and social skills
  • Learning daily living skills
Our Direct Support Professionals receive extensive training as prescribes by State of Minnesota rules and regulations to qualify for In-Home Support Services you need MR/RE waiver funding.

Eligibility Requirements for MR/RC Wavered services:
  1. Has mental retardation or a related condition

  2. Requires a 24-hour plan of care

  3. Is a resident of an ICF/MR or needs the level of services found in an ICF/MR if the waiver services were not available

  4. Meets income and asset requirements for Medical Assistance (MA), expect for children enrolled in ht TEFRA program who become eligible regardless of family income

  5. Has made and informed choice requesting MR/RC waiver services instead of ICF/MR services.

How to obtain Wavered Service:

A person interested in receiving wavered services must contact his/her local county Children and Adult family services. This county must appoint a case manager within ten working days after receiving an application for services. Once a case manager is appointed, he/she is responsible to coordinate the completion of a comprehensive diagnostic evaluation within thirty-five (35) working days after receiving an application for services. This diagnostic evaluation is used to determine whether the person has a diagnosis of mental retardation or a related condition and is therefore eligible to receive MR/RC wavered services.
